Colborne Rural Outreach Clinic
The Colborne Rural Outreach Clinic (ROC) is the first clinic of its kind organized by the Ontario Health Team of Northumberland (OHT-N).
Launched April 6, 2021, this clinic is part of OHT-N efforts to bring services to rural community locations in Northumberland to help reduce barriers to care such as access, transportation and outreach challenges. Colborne ROC services are provided through a collaboration of OHT-N partners and Cramahe Township.
Services are available by appointment only. Walk-in support is not available at the clinic at this time.
Services currently include:
- Primary care with a Nurse Practitioner (for patients who do not currently have a local primary care provider, or those who may need specific temporary support closer to home)
- Adult and youth mental health supports
- Foot care with a Chiropodist
- Diabetes education
Future services planned for this location include:
- Smoking cessation
- Seniors wellness supports
