What is OHT-N?

The Ontario Health Team of Northumberland (OHT-N) is one of 58 Ontario Health Teams across the province, introduced in 2019 to help build a more connected and sustainable health care system centred on the needs of patients and caregivers.

Funded by the Government of Ontario, OHT-N is a county-wide collaboration of patients, caregivers, health care providers, and community and social service organizations working together to co-design and deliver a more integrated model of care.

By bringing partners together as one coordinated team, OHT-N aims to reduce complexity across the health system and improve access to a seamless continuum of services—including primary care, acute care, home and community care, mental health and addictions, palliative care, and emergency services.

Through this approach, OHT-N is focused on improving patient and population health outcomes, enhancing the experience of patients, families, caregivers, and providers, delivering greater value, and advancing health equity across Northumberland. 2022-2026 Strategic Plan

Northumberland County has a higher senior population per capita than the provincial average, and many residents live in rural areas. As a result, the OHT-N's initial focus has been to coordinate health care services for older adults living with complex conditions and rural populations—specifically, those who experience barriers to care.

The OHT-N 2022-2026 Strategic Plan sets our direction for shaping, together, the future of care and services for Northumberland. OHT-N partners will focus on advancing the following health and well-being priorities for Northumberland:

  • Improving access to primary and specialty care and services in our region.
  • Supporting older adults with complex conditions to live and age well at home.
  • Improving access and services for those who have mental health and addiction needs.
