Colborne Rural Outreach Clinic
The Colborne Rural Outreach Clinic (ROC) is a community-focused healthcare initiative by the Ontario Health Team of Northumberland (OHT-N), launched on April 6, 2021. Designed to improve healthcare access for rural residents, the clinic addresses barriers such as transportation and service availability by bringing services closer to residents. Through partnerships with OHT-N affiliates and Cramahe Township, the ROC connects patients with essential primary care, mental health services, and other supports—helping to bridge the gap in rural healthcare.

Booking an Appointment
At our clinic, appointments are scheduled based on a triage process to ensure that all patients receive appropriate care. Requests are reviewed by our team and prioritized according to individual healthcare needs, which means appointments may sometimes be subject to a waitlist. We appreciate your patience and understanding as we work to meet the needs of all our community in a timely manner.
Services currently include
- Interim primary care with Nurse Practitioner (for patients who do not currently have a primary care provider)
- Adult and youth mental health supports
- Diabetes education
- Family violence prevention resources and support
- Community care support with nutrition, transportation and other essential services to help maintain independence
Program availability may change based on the availability of our community partners. Specialized services offered by our partners are scheduled on a rotating basis and are not available every day of the week. This rotating schedule ensures we can provide a wide range of healthcare services to meet the diverse needs of our community. Please call us for specific service availability. Additionally, due to high demand, new registrants may be placed on a waitlist for services.
