Primary Care Network

The purpose of the Ontario Health Team of Northumberland Primary Care Network (PCN) is for providers and clinicians in primary care to communicate with each other, share resources and knowledge, and collaborate, while also allowing the PCN to speak as one voice if the need arises. Through its collective voice, the PCN can help break down health system barriers, address inequities in health outcomes and access in local communities and lead primary care planning activities that will improve patient experiences.

Vision of the Primary Care Network

The PCN will connect, integrate, and support primary care providers within OHTs to improve the delivery and coordination of care for patients.
