Help Shape the Future of Health Care in Northumberland

The Ontario Health Team of Northumberland (OHT-N) is developing its next five-year Strategic Plan, and we want to hear from you.

Whether you've visited a family doctor, received care in the hospital, accessed community support services, cared for a loved one, or simply live in Northumberland County, your experiences and ideas matter. Your feedback will help us better understand the health care needs, challenges and priorities of our community as we plan for the future.

Why Your Feedback Matters

The OHT-N brings together health care providers, community organizations, patients, caregivers, and partners to improve how health and social services are planned and delivered across Northumberland County.

As we develop our next Strategic Plan, we're looking to residents to help identify:

  • The health and wellness needs that matter most to our community.
  • Barriers people face when accessing care and services.
  • Opportunities to strengthen partnerships and improve coordination across the health system.
  • Priorities that should guide OHT-N's work over the next five years.

Your input will help ensure our Strategic Plan reflects the voices and experiences of the people who live, work, and receive care in Northumberland.

Take the Survey

The survey takes approximately 10 to 15 minutes to complete, and your responses will remain confidential.

The survey closes on Friday, September 4.

Thank you for taking the time to share your feedback. Every response helps us better understand the needs of our community and will contribute to shaping a healthier future for everyone in Northumberland.
